Due to the different laws and statutes there is no “one” consent form template that would be legally correct. We are unable to provide you with a template, however, we can give you the exact wording that should be included somewhere within the form that you do use.


Along with the other terms in your consent form, performers must also consent to the following:


  • Consent to be depicted in the content; and 
  • Consent to allow for the public distribution of the content; and 
  • Consent to upload the content to Clips4Sale.com; and 
  • Consent to have the content downloaded. 

 

NOTE: You do not have to state Clips4Sale.com in your release, but it’s a good idea to let them know where their image may be displayed/listed for sale.
